Joining the dots does not appear to happen here at this surgery, at least not in my case. Unless there is an overall assessment of chronic, diagnosed health issues and a planned strategy, agreed with the patient, there is lack of Continuing Care Management. Visits to the surgery appear to be seen as acute and not in the context of a Continuing Care Management Strategy. The use of locum doctors adds to the problems, causing frustration and loss of confidence and as in my case making the decision not to visit the surgery unless I really have no choice. A patient cannot self manage, without an overall medical strategy, that may or may not include other professionals, as their chronic conditions may be complex and unstable.This leads to a complete lack of cohesion between all parties. Assessment, strategy and outcomes are the essential requirements for many professions Perhaps more than essential in medicine.
