Attended and initially saw Nurse Amanda, who while talking to me expanded the discussion into a social care issue that was impacting on my health. Nurse Amanda referred me the Social Prescriber assigned to the GP Practice who was able to help me solve my social care issue. Fantastic demonstration of social and medical practice working together to provide a slick function. Brilliant, thank you. ?? | ||||||
