Care Management Group 78 Stubbington Lane

The provider is registered to provide the following regulated activity: Accommodation for persons who require nursing or personal care. The service provides accommodation and personal care for up to five people who have learning disabilities or autistic spectrum disorder. This service is not registered to provide nursing care. The service has a registered manager.

Stubbington Lane is a specialist residential service for adults with learning disabilities, mental health needs and challenging behaviour.

Service users are supported to develop individualised person centred plans that promote increased choice, control over their lives and independence, whilst focusing on recovery and positive behaviour support.

Our experienced team works in partnership with families, carers, advocates and public service professionals to ensure that the support provided is pro-active and appropriate.

We follow an active, person centred approach together with the principles of Total Communication, which supports service users to make choices and achieve their planned outcomes.

We work closely with community teams to ensure that tenants continue to access key services and appointments, and this close working relationship enables us to focus on increased independence and recovery for service users who have mental health needs.

We actively encourage personalisation and welcome enquiries and self referrals from service users and their supporters in addition to referrals from local authority teams.
